T\u00e1c \u0111\u1ed9ng c\u1ee7a tia x\u1ea1 l\u00ean c\u00e1c m\u00f4<\/h2>\n<h3>2.1 T\u00e1c \u0111\u1ed9ng l\u00ean m\u00f4 b\u01b0\u1edbu:<\/h3>\n<p>V\u1eadn t\u1ed1c t\u0103ng tr\u01b0\u1edfng c\u1ee7a m\u00f4 b\u01b0\u1edbu t\u00f9y thu\u1ed9c:<\/p>\n<p>(1) \u00a0 Th\u1eddi gian c\u1ee7a chu k\u00ec t\u1ebf b\u00e0o, trung b\u00ecnh t\u1eeb 2 \u0111\u1ebfn 4 ng\u00e0y, nh\u01b0ng c\u00f3 s\u1ef1 kh\u00e1c bi\u1ec7t r\u1ea5t l\u1edbn gi\u1eefa c\u00e1c t\u1ebf b\u00e0o trong m\u00f4 b\u01b0\u1edbu.<\/p>\n<p>(2) \u00a0 H\u1ec7 s\u1ed1 t\u0103ng tr\u01b0\u1edfng c\u1ee7a b\u01b0\u1edbu, l\u00e0 ph\u1ea7n tr\u0103m s\u1ed1\u00a0 t\u1ebf b\u00e0o \u0111ang ph\u00e2n chia, thay \u0111\u1ed5i t\u1eeb\u2019 0,3% \u0111\u1ebfn 100%.<\/p>\n<p>(3) \u00a0 H\u1ec7\u00a0 s\u1ed1 m\u1ea5t t\u1ebf b\u00e0o (coefficient d\u00ea perte cellulaires), c\u00f3 th\u1ec3\u2019 l\u00ean t\u1edbi 90%, do s\u1ef1 bi\u1ec7t h\u00f3a t\u1ebf b\u00e0o, do s\u1ed1 t\u1ebf b\u00e0o ch\u1ebft trong l\u00fac ph\u00e2n b\u00e0o v\u00e0 do s\u1ef1 di d\u00e2n c\u1ee7a t\u1ebf b\u00e0o ra kh\u1ecfi b\u01b0\u1edbu.<\/p>\n<p>X\u00e1c su\u1ea5t qu\u00e9t s\u1ea1ch c\u00e1c t\u1ebf b\u00e0o sinh clon t\u00f9y thu\u1ed9c nhi\u1ec1u y\u1ebfu t\u1ed1, trong \u0111\u00f3 quan tr\u1ecdng nh\u1ea5t l\u00e0:<\/p>\n<p>(1) \u00a0 <em>Li\u1ec1u tia:<\/em> m\u1ee9c \u0111\u1ed9 ti\u00eau di\u1ec7t b\u01b0\u1edbu t\u0103ng d\u1ea7n theo li\u1ec1u tia trong m\u1ed9t gi\u1edbi h\u1ea1n\u00a0 n\u00e0o \u0111\u00f3, v\u00e0 c\u00e1c nghi\u00ean c\u1ee9u c\u0169ng cho th\u1ea5y th\u01b0\u1eddng ch\u1ec9 c\u1ea7n t\u0103ng th\u00eam v\u00e0i Gy th\u00ec m\u1ee9c \u0111\u1ed9 ti\u00eau di\u1ec7t b\u01b0\u1edbu c\u0169ng c\u00f3 th\u1ec3 t\u0103ng t\u1eeb 50% l\u00ean \u0111\u1ebfn 90%.<\/p>\n<p>(2) \u00a0 <em>Kh\u1ed1i l\u01b0\u1ee3ng b\u01b0\u1edbu:<\/em> c\u00e1c b\u01b0\u1edbu l\u1edbn th\u01b0\u1eddngkho ti\u00eau di\u1ec7t h\u01a1n c\u00e0c b\u01b0\u1edbu nh\u1ecf; v\u00e0 li\u1ec1u tia \u0111\u1ec3 qu\u00e9t s\u1ea1ch c\u00e1c t\u1ebf b\u00e0o b\u01b0\u1edbu x\u00e2m l\u1ea5n vi th\u1ec3 th\u1ea5p h\u01a1n li\u00eau tia c\u1ea7n cho b\u01b0\u1edbu \u0111\u00e3 \u1edf m\u1ee9c \u0111\u1ed9 ph\u00e1t hi\u1ec7n \u0111\u01b0\u1ee3c tr\u00ean l\u00e2m s\u00e0ng.<\/p>\n<p>(3) \u00a0 <em>Lo\u1ea1i gi\u1ea3i ph\u1eabu b\u1ec7nh:<\/em> m\u1ed9t s\u1ed1 b\u01b0\u1edbu r\u1ea5t nh\u1ea1y tia (seminoma, lympho \u00e1c &#8230;), m\u1ed9t s\u1ed1 b\u01b0\u1edbu nh\u1ea1y tia trung b\u00ecnh (h\u1ea7u h\u1ebft c\u00e1c carcinom), va cac b\u01b0\u1edbu \u00edt nh\u1ea1y tia (m\u00ealan\u00f4m, sarcom&#8230;).<\/p>\n<h3>2.2 T\u00e1c \u0111\u1ed9ng l\u00ean m\u00f4 l\u00e0nh<\/h3>\n<p>C\u00f3 hai lo\u1ea1i t\u00e1c \u0111\u1ed9ng \u0111\u1ebfn m\u00f4 l\u00e0nh<\/p>\n<p>(1) \u00a0 C\u00e1c ph\u1ea3n \u1ee9ng s\u1edbm xu\u1ea5t hi\u1ec7n v\u00e0i ng\u00e0y hay v\u00e0i tu\u1ea7n sau xa tr\u1ecb, dos\u1ef1 ch\u1ebft \u0111i m\u1ed9t s\u1ed1 l\u01b0\u1ee3ng l\u1edbn t\u1ebf b\u00e0o trong m\u00f4 c\u00f3 s\u1ef1 t\u0103ng tr\u01b0\u1edfng nhanh, s\u1ebd ph\u1ee5c h\u1ed3i sau v\u00e0i tu\u1ea7n do s\u1ef1 t\u00e1i l\u1eadp s\u1ed1 l\u01b0\u1ee3ng\u00a0 t\u1eeb c\u00e1c t\u1ebf b\u00e0o m\u1ea7m c\u00f2n l\u1ea1i.<\/p>\n<p>(2) \u00a0 C\u00e1c ph\u1ea3n \u1ee9ng mu\u1ed9n xu\u1ea5t hi\u1ec7n v\u00e0i th\u00e1ng \u0111\u1ebfn nhi\u1ec1u n\u0103m sau, v\u00e0 do t\u00e1c \u0111\u1ed9ng l\u00ean c\u00e1c m\u00f4 ph\u00e1t tri\u1ec3n ch\u1eadm.<\/p>\n<p>T\u1ea5t c\u1ea3\u00a0 bi\u1ebfn ch\u1ee9ng ph\u1ea3i \u0111\u01b0\u1ee3c d\u1ef1 ki\u1ebfn va gi\u1ea3m thi\u1ec3u t\u1ed1i \u0111a khi l\u1eadp k\u1ebf ho\u1ea1ch x\u1ea1 tr\u1ecb.<\/p>\n<h2>3. C\u00e1c ph\u01b0\u01a1ng ph\u00e1p x\u1ea1 tr\u1ecb trong ung th\u01b0<\/h2>\n<p><strong><em>X\u1ea1 tr\u1ecb ngo\u00e0i<\/em><\/strong><\/p>\n<p><em>\u0110\u1ecbnh ngh\u0129a c\u00e1c kh\u1ed1i l\u01b0\u1ee3ng m\u00f4 \u0111\u01b0\u1ee3c x\u1ea1 tri<\/em><\/p>\n<p>Th\u1ec3 t\u00edch b\u01b0\u1edbuto\u00e0n th\u1ec3 \u2019 (Gross Tumor Volume-GTV) l\u00e0 kh\u1ed1i l\u01b0\u1ee3ng b\u01b0\u1edbu tr\u00ean l\u00e2m s\u00e0ng k\u1ec3 c\u1ea3 c\u00e1c h\u1ea1ch v\u00f9ng.<\/p>\n<p>Th\u1ec3 t\u00edch x\u1ea1 l\u00e2m s\u00e0ng (Clinical Target Volume-CTV) g\u1ed3m GTV va m\u1ed9t r\u00eca b\u00ean ngo\u00e0i bao tr\u00f9m c\u00e1c ph\u1ea7n b\u01b0\u1edbu vi th\u1ec3.<\/p>\n<p>Th\u1ec3 t\u00edch x\u1ea1 k\u1ebf ho\u1ea1ch (Planning Target Volume-PTV) g\u1ed3m CTV va m\u1ed9t v\u00e0nh b\u00ean ngo\u00e0i d\u00e0nh cho s\u1ef1 sai s\u1ed1 khi \u0111\u1eb7t tr\u01b0\u1eddng chi\u1ebfu v\u00e0 s\u1ef1 di \u0111\u1ed9ng khi \u0111i\u1ec1u tr\u1ecb nh\u01b0 s\u1ef1 di \u0111\u1ed9ng theo nh\u1ecbp th\u1edf.<\/p>\n<p><em>C\u00e1c tr\u01b0\u1eddng chi\u1ebfu<\/em><\/p>\n<p>Tr\u01b0\u1eddng chi\u1ebfu \u0111\u01a1n: sang th\u01b0\u01a1ng ngo\u00e0i da, nong d\u01b0\u1edbi da (h\u1ea1ch c\u1ed5, n\u00e1ch, b\u1eb9n).<\/p>\n<p>Tr\u01b0\u1eddng chi\u1ebfu \u0111\u00f4i song song: sang th\u01b0\u01a1ng \u1edf tr\u1ee5c c\u01a1 th\u1ec3, n\u1eb1m s\u00e2u g\u1ea7n m\u1eb7t ph\u1eb3ng gi\u1eefa c\u01a1 th\u1ec3 (t\u1eed cung, ph\u1ed5i&#8230;).<\/p>\n<p>Tr\u01b0\u1eddng chi\u1ebfu k\u1ebft h\u1ee3p: 3 tr\u01b0\u1eddng chi\u1ebfu (v\u00f2m h\u1ea7u), 4 tr\u01b0\u1eddng chi\u1ebfu (v\u00f9ng chi\u1ebfu).<\/p>\n<p><strong><em>X\u1ea1 tr\u1ecb ti\u1ebfp c\u1eadn<\/em><\/strong><\/p>\n<p><em>Ch\u1ec9 \u0111\u1ecbnh<\/em><\/p>\n<p>Ph\u1ed1i h\u1ee3p v\u1edbi x\u1ea1 tr\u1ecb ngo\u00e0i: \u0111\u00e2y l\u00e0 ch\u1ec9 \u0111\u1ecbnh th\u01b0\u1eddng \u0111\u01b0\u1ee3c s\u1eed d\u1ee5ng \u0111\u1ec3 gi\u1ea3m c\u00e1c bi\u1ebfn ch\u1ee9ng l\u00ean
