Theo GLOBOCAN 2018, tr\u00ean th\u1ebf gi\u1edbi s\u1ed1 ca m\u1edbi m\u1eafc c\u1ee7a ung th\u01b0 ph\u1ed5i \u0111\u1ee9ng h\u00e0ng \u0111\u1ea7u v\u1edbi h\u01a1n 2 tri\u1ec7u ng\u01b0\u1eddi\/n\u0103m, t\u1ec9 l\u1ec7 t\u1eed vong c\u0169ng cao nh\u1ea5t trong s\u1ed1 c\u00e1c b\u1ec7nh ung th\u01b0 v\u1edbi 1,76 tri\u1ec7u ng\u01b0\u1eddi\/n\u0103m (18,4%). T\u1ea1i Vi\u1ec7t Nam, t\u00ednh chung c\u1ea3 2 gi\u1edbi, t\u1ec9 l\u1ec7 m\u1edbi m\u1eafc c\u0169ng nh\u01b0 t\u1eed vong c\u1ee7a ung th\u01b0 ph\u1ed5i \u0111\u1ee9ng h\u00e0ng th\u1ee9 nh\u1ea5t v\u1edbi t\u1ec9 l\u1ec7 l\u1ea7n l\u01b0\u1ee3t l\u00e0 14% v\u00e0 19,5%.<\/p>\n<p>1. \u0110\u1ea1i c\u01b0\u01a1ng<\/p>\n<p>Ung th\u01b0 ph\u1ed5i kh\u00f4ng t\u1ebf b\u00e0o nh\u1ecf (NSCLC) \u0111\u01b0\u1ee3c chia th\u00e0nh hai lo\u1ea1i ch\u00ednh d\u1ef1a tr\u00ean t\u00ednh ch\u1ea5t c\u1ee7a v\u1ea9y tr\u00ean b\u1ec1 m\u1eb7t t\u1ebf b\u00e0o ung th\u01b0 \u0111\u00f3 l\u00e0:<\/p>\n<ul>\n<li>NSCLC c\u00f3 v\u1ea9y (squamous cell carcinoma): Lo\u1ea1i ung th\u01b0 n\u00e0y xu\u1ea5t ph\u00e1t t\u1eeb c\u00e1c t\u1ebf b\u00e0o bi\u1ec3u m\u00f4 ph\u1eb3ng tr\u00ean c\u00e1c \u0111\u01b0\u1eddng d\u1eabn kh\u00ed qu\u1ea3n v\u00e0 ph\u1ed5i. \u0110i\u1ec1u n\u00e0y d\u1eabn \u0111\u1ebfn s\u1ef1 h\u00ecnh th\u00e0nh v\u1ea9y \u1edf b\u1ec1 m\u1eb7t c\u1ee7a t\u1ebf b\u00e0o ung th\u01b0.<\/li>\n<li>NSCLC kh\u00f4ng c\u00f3 v\u1ea9y (non-squamous cell carcinoma): Lo\u1ea1i ung th\u01b0 n\u00e0y kh\u00f4ng c\u00f3 \u0111\u1eb7c \u0111i\u1ec3m v\u1ea9y tr\u00ean b\u1ec1 m\u1eb7t t\u1ebf b\u00e0o ung th\u01b0 v\u00e0 bao g\u1ed3m nhi\u1ec1u lo\u1ea1i kh\u00e1c nhau, ch\u1ee7 y\u1ebfu l\u00e0 t\u1ebf b\u00e0o bi\u1ec3u m\u00f4 tuy\u1ebfn v\u00e0 t\u1ebf b\u00e0o l\u1edbn.<\/li>\n<\/ul>\n<figure id=\"attachment_26359\" aria-describedby=\"caption-attachment-26359\" style=\"width: 300px\" class=\"wp-caption aligncenter\"><img decoding=\"async\" loading=\"lazy\" class=\"size-medium wp-image-26359\" src=\"http:\/\/noithathoaphat.in\/ctv\/wp-content\/uploads\/2023\/04\/OIG-2-300x300.jpg\" alt=\"\" width=\"300\" height=\"300\" srcset=\"http:\/\/noithathoaphat.in\/ctv\/wp-content\/uploads\/2023\/04\/OIG-2-300x300.jpg 300w, http:\/\/noithathoaphat.in\/ctv\/wp-content\/uploads\/2023\/04\/OIG-2-150x150.jpg 150w, http:\/\/noithathoaphat.in\/ctv\/wp-content\/uploads\/2023\/04\/OIG-2.jpg 351w\" sizes=\"(max-width: 300px) 100vw, 300px\" \/><figcaption id=\"caption-attachment-26359\" class=\"wp-caption-text\">Ung th\u01b0 ph\u1ed5i \u0111\u01b0\u1ee3c chia th\u00e0nh hai lo\u1ea1i ch\u00ednh d\u1ef1a tr\u00ean t\u00ednh ch\u1ea5t t\u1ebf b\u00e0o: Ung th\u01b0 ph\u1ed5i t\u1ebf b\u00e0o nh\u1ecf v\u00e0 ung th\u01b0 ph\u1ed5i kh\u00f4ng t\u1ebf b\u00e0o nh\u1ecf<\/figcaption><\/figure>\n<p>Vi\u1ec7c ph\u00e2n lo\u1ea1i NSCLC d\u1ef1a tr\u00ean t\u00ednh ch\u1ea5t c\u1ee7a v\u1ea9y gi\u00fap c\u00e1c chuy\u00ean gia y t\u1ebf x\u00e1c \u0111\u1ecbnh ph\u01b0\u01a1ng ph\u00e1p \u0111i\u1ec1u tr\u1ecb ph\u00f9 h\u1ee3p v\u00e0 d\u1ef1 b\u00e1o l\u00e2m s\u00e0ng cho b\u1ec7nh nh\u00e2n.<\/p>\n<p>NSCLC th\u01b0\u1eddng kh\u00f4ng \u0111\u01b0\u1ee3c ph\u00e1t hi\u1ec7n s\u1edbm v\u00ec kh\u00f4ng c\u00f3 tri\u1ec7u ch\u1ee9ng r\u00f5 r\u00e0ng \u1edf giai \u0111o\u1ea1n \u0111\u1ea7u. Tuy nhi\u00ean, c\u00e1c tri\u1ec7u ch\u1ee9ng c\u1ee7a NSCLC c\u00f3 th\u1ec3 bao g\u1ed3m ho khan, kh\u00f3 th\u1edf, \u0111au ng\u1ef1c, s\u1ef1 gi\u1ea3m c\u00e2n v\u00e0 m\u1ec7t m\u1ecfi. Vi\u1ec7c \u0111i\u1ec1u tr\u1ecb NSCLC th\u01b0\u1eddng bao g\u1ed3m ph\u1eabu thu\u1eadt, h\u00f3a tr\u1ecb v\u00e0\/ho\u1eb7c x\u1ea1 tr\u1ecb, ph\u1ee5 thu\u1ed9c v\u00e0o giai \u0111o\u1ea1n c\u1ee7a b\u1ec7nh v\u00e0 t\u00ecnh tr\u1ea1ng s\u1ee9c kh\u1ecfe c\u1ee7a b\u1ec7nh nh\u00e2n.<\/p>\n<p>2. \u0110i\u1ec1u tr\u1ecb ung th\u01b0 ph\u1ed5i kh\u00f4ng t\u1ebf b\u00e0o nh\u1ecf<\/p>\n<p>Hai m\u1ee5c ti\u00eau ch\u00ednh trong \u0111i\u1ec1u tr\u1ecb ung th\u01b0 ph\u1ed5i kh\u00f4ng t\u1ebf b\u00e0o nh\u1ecf ph\u00e2n theo giai \u0111o\u1ea1n: \u0111i\u1ec1u tr\u1ecb tri\u1ec7t c\u0103n nh\u1eb1m m\u1ee5c ti\u00eau ch\u1eefa kh\u1ecfi \u0111\u01b0\u1ee3c ch\u1ec9 \u0111\u1ecbnh cho b\u1ec7nh \u1edf giai \u0111o\u1ea1n s\u1edbm khu tr\u00fa t\u1ea1i ch\u1ed7, t\u1ea1i v\u00f9ng (giai \u0111o\u1ea1n I,II, IIIA) v\u00e0 \u0111i\u1ec1u tr\u1ecb mang t\u00ednh gi\u1ea3m nh\u1eb9, t\u0103ng ch\u1ea5t l\u01b0\u1ee3ng cu\u1ed9c s\u1ed1ng v\u00e0 k\u00e9o d\u00e0i th\u1eddi gian s\u1ed1ng th\u00eam \u00e1p d\u1ee5ng cho giai \u0111o\u1ea1n ti\u1ebfn tri\u1ec3n t\u1ea1i ch\u1ed7 &#8211; t\u1ea1i v\u00f9ng ho\u1eb7c t\u00e1i ph\u00e1t di c\u0103n (giai \u0111o\u1ea1n IIIB, IV). \u0110\u1ed1i v\u1edbi giai \u0111o\u1ea1n c\u00f2n khu tr\u00fa, ph\u1eabu thu\u1eadt v\u00e0 x\u1ea1 tr\u1ecb l\u00e0 ph\u01b0\u01a1ng ph\u00e1p \u0111i\u1ec1u tr\u1ecb mang t\u00ednh tri\u1ec7t c\u0103n. Giai \u0111o\u1ea1n ti\u1ebfn tri\u1ec3n (t\u00e1i ph\u00e1t\/di c\u0103n) vi\u1ec7c \u0111i\u1ec1u tr\u1ecb ch\u1ee7 y\u1ebfu mang t\u00ednh l\u00e0m d\u1ecbu tri\u1ec7u ch\u1ee9ng v\u00e0 k\u00e9o d\u00e0i th\u1eddi gian s\u1ed1ng th\u00eam. Ph\u1eabu thu\u1eadt v\u00e0 x\u1ea1 tr\u1ecb mang t\u00ednh c\u00e1ch tri\u1ec7t \u0111\u1ec3, vai tr\u00f2 c\u1ee7a h\u00f3a tr\u1ecb b\u1ed5 tr\u1ee3 tr\u01b0\u1edbc v\u00e0 sau \u0111i\u1ec1u tr\u1ecb tri\u1ec7t \u0111\u1ec3 c\u00f2n khi\u00eam t\u1ed1n.<\/p>\n<ul>\n<li>Ph\u1eabu thu\u1eadt: y\u00eau c\u1ea7u c\u1ee7a ph\u1eabu thu\u1eadt tri\u1ec7t \u0111\u1ec3 bao g\u00f4\u0300m vi\u1ec7c c\u1eaft tr\u1ecdn th\u00f9y ph\u1ed5i mang kh\u1ed1i u \u0111\u1ea1t di\u1ec7n c\u1eaft \u00e2m t\u00ednh (kh\u00f4ng c\u00f3 t\u1ebf b\u00e0o b\u01b0\u1edbu), l\u1ea5y \u0111i t\u1eeb 16-20 h\u1ea1ch thu\u1ed9c \u00edt nh\u1ea5t 3 nh\u00f3m h\u1ea1ch (2 t\u1eeb trung th\u1ea5t c\u00f9ng v\u00e0 \u0111\u1ed1i b\u00ean) v\u00e0 r\u1eefa m\u00e0ng ph\u1ed5i c\u00f9ng b\u00ean. Nguy c\u01a1 t\u1eed vong v\u00e0o kho\u1ea3ng 3% sau ph\u1eabu thu\u1eadt c\u1eaft th\u00f9y v\u00e0 7% sau ph\u1eabu thu\u1eadt c\u1eaft m\u1ed9t b\u00ean ph\u1ed5i. C\u00e1c bi\u1ec7n ph\u00e1p ph\u1eabu thu\u1eadt bao g\u00f4\u0300m: c\u1eaft h\u00ecnh ch\u00eam, c\u1eaft ph\u00e2n th\u00f9y, c\u1eaft 1 th\u00f9y ph\u1ed5i, c\u1eaft 2 th\u00f9y ph\u1ed5i ho\u1eb7c c\u1eaft to\u00e0n b\u1ed9 m\u1ed9t l\u00e1 ph\u1ed5i k\u00e8m theo n\u1ea1o v\u00e9t h\u1ea1ch v\u00f9ng tri\u1ec7t c\u0103n.<\/li>\n<li>H\u00f3a tr\u1ecb b\u1ed5 tr\u1ee3 sau m\u1ed5 \u0111\u00e3 \u0111\u01b0\u1ee3c nghi\u00ean c\u1ee9u nhi\u1ec1u v\u00e0 c\u00e1c nghi\u00ean c\u1ee9u cho k\u1ebft qu\u1ea3 kh\u00e1c nhau. H\u00f3a tr\u1ecb b\u1ed5 tr\u1ee3 sau ph\u1eabu thu\u1eadt hi\u1ec7n \u0111\u01b0\u1ee3c ch\u1ec9 \u0111\u1ecbnh cho c\u00e1c tr\u01b0\u1eddng h\u1ee3p c\u00f3 giai \u0111o\u1ea1n t\u1eeb IB tr\u1edf l\u00ean. X\u1ea1 tr\u1ecb b\u1ed5 tr\u1ee3 sau ph\u1eabu thu\u1eadt giai \u0111o\u1ea1n I v\u00e0 II kh\u00f4ng \u0111\u01b0\u1ee3c x\u00e1c nh\u1eadn c\u00f3 \u00edch l\u1ee3i. Trong th\u1ef1c h\u00e0nh l\u00e2m s\u00e0ng, x\u1ea1 tr\u1ecb b\u1ed5 tr\u1ee3 th\u01b0\u1eddng \u0111\u01b0\u1ee3c ch\u1ec9 \u0111\u1ecbnh sau m\u1ed5 cho m\u1ed9t s\u1ed1 tr\u01b0\u1eddng h\u1ee3p c\u00f3 y\u1ebfu t\u1ed1 nguy c\u01a1 cao t\u00e1i ph\u00e1t t\u1ea1i ch\u1ed7, t\u1ea1i v\u00f9ng nh\u01b0: x\u00e2m l\u1ea5n trung th\u1ea5t, di c\u0103n nhi\u1ec1u h\u1ea1ch, h\u1ea1ch di c\u0103n c\u00f3 k\u00edch th\u01b0\u1edbc l\u1edbn, x\u00e2m l\u1ea5n v\u1ee1 v\u1ecf bao h\u1ea1ch, b\u1edd di\u1ec7n c\u1eaft c\u00f2n t\u1ebf b\u00e0o ung th\u01b0.<\/li>\n<\/ul>\n<p>M\u1ed9t s\u1ed1 tr\u01b0\u1eddng h\u1ee3p\u00a0 giai \u0111o\u1ea1n c\u00f2n khu tr\u00fa nh\u01b0ng kh\u00f4ng th\u1ec3 ti\u1ebfn h\u00e0nh ph\u1eabu thu\u1eadt tri\u1ec7t c\u0103n do nguy c\u01a1 ph\u1eabu thu\u1eadt cao, c\u00f3 th\u1ec3 c\u00e2n nh\u1eafc \u00e1p d\u1ee5ng m\u1ed9t s\u1ed1 bi\u1ec7n ph\u00e1p kh\u00e1c. X\u1ea1 tr\u1ecb tri\u1ec7t c\u0103n l\u00e0 m\u1ed9t l\u1ef1a ch\u1ecdn thay th\u1ebf, c\u00f3 th\u1ec3 ti\u1ebfn h\u00e0nh theo k\u1ef9 thu\u1eadt x\u1ea1 tr\u1ecb quy \u01b0\u1edbc, t\u1ef7 l\u1ec7 ki\u1ec3m so\u00e1t b\u01b0\u1edbu kho\u1ea3ng 50% ho\u1eb7c x\u1ea1 tr\u1ecb \u0111\u1ecbnh v\u1ecb th\u00e2n (<a href=\"https:\/\/www.vinmec.com\/vi\/ung-buou-xa-tri\/thong-tin-suc-khoe\/xa-tri-cong-nghe-cao-ky-thuat-xa-phau-dinh-vi-sbrt\/\">SBRT: stereostatic body radiotherapy<\/a>) hi\u1ec7u qu\u1ea3 ki\u1ec3m so\u00e1t b\u01b0\u1edbu t\u1ea1i ch\u1ed7 l\u00ean t\u1edbi 85%-90%.<\/p>\n<p>2.2. \u0110i\u1ec1u tr\u1ecb giai \u0111o\u1ea1n ti\u1ebfn tri\u1ec3n t\u1ea1i ch\u1ed7 (giai \u0111o\u1ea1n III)<\/p>\n<p>Hi\u1ec7n nay, bi\u1ec7n ph\u00e1p \u0111i\u1ec1u tr\u1ecb hi\u1ec7u qu\u1ea3 nh\u1ea5t cho giai \u0111o\u1ea1n n\u00e0y v\u1eabn chua \u0111u\u1ee3c x\u00e1c \u0111\u1ecbnh v\u00e0 c\u00f2n tranh c\u00e3i tr\u00ean nhi\u1ec1u phu\u01a1ng di\u1ec7n v\u00ec tuy c\u00f3 nhi\u1ec1u l\u1ef1a ch\u1ecdn kh\u00e1c nhau nhung chua c\u00f3 bi\u1ec7n ph\u00e1p n\u00e0o cho th\u1ea5y kh\u1ea3 nang \u0111i\u1ec1u tr\u1ecb kh\u1ecfi cao.<\/p>\n<p>Nguy\u00ean t\u1eafc \u0111i\u1ec1u tr\u1ecb giai \u0111o\u1ea1n n\u00e0y th\u01b0\u1eddng \u0111\u01b0\u1ee3c c\u00e1 th\u1ec3 h\u00f3a theo t\u1eebng tr\u01b0\u1eddng h\u1ee3p b\u1ec7nh nh\u00e2n c\u1ee5 th\u1ec3 v\u1edbi nhi\u1ec1u m\u00f4 th\u1ee9c \u00e1p d\u1ee5ng l\u1ea7n l\u01b0\u1ee3t hay \u0111\u00f4\u0300ng th\u1eddi. Ph\u1eabu thu\u1eadt c\u00f3 th\u1ec3 th\u1ef1c hi\u1ec7n \u1edf m\u1ed9t s\u1ed1 tr\u01b0\u1eddng h\u1ee3p giai \u0111o\u1ea1n IIIA: b\u01b0\u1edbu T3-4 ngo\u1ea1i bi\u00ean, c\u00f2n khu tr\u00fa, h\u1ea1ch N1-2 (nh\u1ecf) tuy nhi\u00ean ph\u1ee5 thu\u1ed9c v\u00e0o tr\u00ecnh \u0111\u1ed9 ph\u1eabu thu\u1eadt vi\u00ean, trang thi\u1ebft b\u1ecb c\u1ee7a c\u01a1 s\u1edf y t\u1ebf, nh\u01b0ng k\u1ebft qu\u1ea3 s\u1ed1ng th\u00eam v\u1eabn c\u00f2n th\u1ea5p.<\/p>\n<p>X\u1ea1 tr\u1ecb \u0111\u00e3 t\u1eebng \u0111\u01b0\u1ee3c xem l\u00e0 m\u00f4 th\u1ee9c \u0111i\u1ec1u tr\u1ecb chu\u1ea9n cho NSCLC giai \u0111o\u1ea1n IIIA\/IIIB nh\u01b0ng k\u1ebft qu\u1ea3 ki\u1ec3m so\u00e1t t\u1ea1i ch\u1ed7 th\u01b0\u1eddng k\u00e9m v\u00e0 t\u1ef7 l\u1ec7 s\u1ed1ng c\u00f2n th\u1ea5p. C\u00e1c nghi\u00ean c\u1ee9u l\u00e2m s\u00e0ng cho th\u1ea5y x\u1ea1 tr\u1ecb \u0111\u01a1n thu\u1ea7n c\u00f3 t\u00e1c d\u1ee5ng l\u00e0m\u00a0 gi\u1ea3m nh\u1eb9 b\u1ec7nh, t\u1ec9 l\u1ec7 s\u1ed1ng sau 5 n\u0103m l\u00e0 r\u1ea5t th\u1ea5p, tuy nhi\u00ean k\u1ebft h\u1ee3p ho\u00e1 ch\u1ea5t v\u1edbi tia x\u1ea1 s\u1ebd gi\u1ea3m \u0111\u01b0\u1ee3c 10% t\u1ec9 l\u1ec7 t\u1eed vong so v\u1edbi nh\u00f3m x\u1ea1 tr\u1ecb \u0111\u01a1n thu\u1ea7n.
