N\u1ebfu PaO2 <\/sub>t\u1eeb 60 \u2013 < 100 mmHg: oxy m\u00e1u gi\u1ea3m \u0111\u00e3 \u0111i\u1ec1u ch\u1ec9nh \u0111\u01b0\u1ee3c nh\u01b0ng b\u1ec7nh nhi s\u1ebd b\u1ecb gi\u1ea3m oxy m\u00e1u n\u1ebfu gi\u1ea3m FiO2<\/sub> .<\/p>\n

Tai bi\u1ebfn khi d\u00f9ng oxy li\u1ec7u ph\u00e1p<\/em><\/p>\n

– \u0110\u1ed1i v\u1edbi tr\u1ebb s\u01a1 sinh d\u1ec5 g\u00e2y x\u1eb9p ph\u1ed5i, t\u1ed5n th\u01b0\u01a1ng t\u1ebf b\u00e0o bi\u1ec3u m\u00f4 ph\u1ed5i, t\u1ed5n th\u01b0\u01a1ng n\u1ed9i m\u1ea1c mao m\u1ea1ch. PaO2<\/sub> cao \u2192 x\u01a1 teo v\u00f5ng m\u1ea1c g\u00e2y m\u00f9 m\u1eaft. \u0110\u1ed1i v\u1edbi tr\u1ebb s\u01a1 sinh thi\u1ebfu th\u00e1ng, PaO2<\/sub> kh\u00f4ng n\u00ean qu\u00e1 75 mmHg.<\/p>\n

– Ng\u01b0\u1eddi l\u1edbn: khi FiO2 <\/sub>\u00a00,80 \u2013 1,00 > 8 gi\u1edd g\u00e2y k\u00edch th\u00edch \u0111\u01b0\u1eddng h\u00f4 h\u1ea5p, \u0111au ng\u1ef1c, x\u01a1 h\u00f3a ph\u1ed5i.<\/p>\n

– B\u1ec7nh nh\u00e2n b\u1ec7nh ph\u1ed5i t\u1eafc ngh\u1ebdn m\u1ea1n t\u00ednh (COPD): d\u1ec5 g\u00e2y \u1ee9 \u0111\u1ecdng CO2, <\/sub>\u1ee9c ch\u1ebf h\u00f4 h\u1ea5p, toan h\u00f4 h\u1ea5p\u2026N\u00ean b\u1eaft \u0111\u1ea7u v\u1edbi FiO2 <\/sub>24%, t\u1ed1i \u0111a l\u00e0 35%. C\u1ea7n gi\u1eef PaO2 <\/sub>\u1edf b\u1ec7nh nh\u00e2n n\u00e0y kho\u1ea3ng 60 mmHg l\u00e0 \u0111\u1ee7.<\/p>\n

2.3. Cung c\u1ea5p n\u0103ng l\u01b0\u1ee3ng v\u00e0 \u0111i\u1ec1u tr\u1ecb h\u1ed7 tr\u1ee3<\/em><\/strong><\/h3>\n

T\u00f9y theo m\u1ee9c \u0111\u1ed9 SHH, t\u00ecnh tr\u1ea1ng c\u1ee7a b\u1ec7nh nhi, vi\u1ec7c cung c\u1ea5p n\u0103ng l\u01b0\u1ee3ng c\u00f3 th\u1ec3 theo c\u00e1c \u0111\u01b0\u1eddng sau:<\/p>\n

– SHH n\u1eb7ng: cung c\u1ea5p n\u0103ng l\u01b0\u1ee3ng qua \u0111\u01b0\u1eddng t\u0129nh m\u1ea1ch, d\u1ecbch th\u01b0\u1eddng d\u00f9ng l\u00e0 Glucose \u01b0u tr\u01b0\u01a1ng. Ch\u00fa \u00fd\u00a0 n\u00ean duy tr\u00ec t\u1ec9 l\u1ec7 lipid v\u00e0 glucid l\u00e0 1\/1.<\/p>\n

– SHH trung b\u00ecnh: cung c\u1ea5p n\u0103ng l\u01b0\u1ee3ng qua \u0111\u01b0\u1eddng mi\u1ec7ng. N\u1ebfu tr\u1ebb b\u00fa k\u00e9m ho\u1eb7c \u0103n u\u1ed1ng k\u00e9m, c\u1ea7n cung c\u1ea5p n\u0103ng l\u01b0\u1ee3ng qua sonde d\u1ea1 d\u00e0y. N\u00ean nh\u1ecf gi\u1ecdt li\u00ean t\u1ee5c v\u00e0 chia th\u00e0nh nhi\u1ec1u c\u1eef\/24 gi\u1edd \u0111\u1ec3 tr\u00e1nh c\u0103ng d\u1ea1 d\u00e0y \u0111\u1ed9t ng\u1ed9t g\u00e2y tr\u00e0o ng\u01b0\u1ee3c, c\u1ea3n tr\u1edf ho\u1ea1t \u0111\u1ed9ng c\u1ee7a c\u01a1 ho\u00e0nh l\u00e0m cho t\u00ecnh tr\u1ea1ng SHH ch\u1eadm c\u1ea3i thi\u1ec7n.<\/p>\n
