D\u1ecbch t\u1ec5 h\u1ecdc<\/b><\/h3>\n<p>Ung th\u01b0 bi\u1ec3u m\u00f4 t\u1ebf b\u00e0o gan l\u00e0 lo\u1ea1i ung th\u01b0 th\u01b0\u1eddng g\u1eb7p nh\u1ea5t v\u00e0 l\u00e0 nguy\u00ean nh\u00e2n h\u00e0ng \u0111\u1ea7u g\u00e2y t\u1eed vong li\u00ean quan \u0111\u1ebfn ung th\u01b0 t\u1ea1i Vi\u1ec7t Nam theo \u01b0\u1edbc t\u00ednh c\u1ee7a T\u1ed5 ch\u1ee9c Ghi nh\u1eadn Ung th\u01b0 to\u00e0n c\u1ea7u (GLOBOCAN) n\u0103m 2018. T\u1ea1i Vi\u1ec7t Nam ch\u01b0a c\u00f3 s\u1ed1 li\u1ec7u qu\u1ed1c gia \u0111\u01b0\u1ee3c c\u00f4ng b\u1ed1 ch\u00ednh th\u1ee9c v\u1ec1 xu\u1ea5t \u0111\u1ed9 ung th\u01b0 bi\u1ec3u m\u00f4 t\u1ebf b\u00e0o gan. M\u1ed9t nghi\u00ean c\u1ee9u ghi nh\u1eadn s\u1ed1 li\u1ec7u ung th\u01b0 bi\u1ec3u m\u00f4 t\u1ebf b\u00e0o gan t\u1ea1i mi\u1ec1n Trung v\u00e0 mi\u1ec1n Nam Vi\u1ec7t Nam l\u00e0 24091 tr\u01b0\u1eddng h\u1ee3p trong th\u1eddi gian 2010 \u0111\u1ebfn 2016, trong \u0111\u00f3 62,3% c\u00f3 nhi\u1ec5m virus vi\u00eam gan B (HBV) m\u1ea1n v\u00e0 26% c\u00f3 nhi\u1ec5m virus vi\u00eam gan C (HCV) m\u1ea1n. Vi\u1ec7t Nam l\u00e0 n\u01b0\u1edbc c\u00f3 xu\u1ea5t \u0111\u1ed9 nhi\u1ec5m HBV cao, \u01b0\u1edbc t\u00ednh c\u00f3 kho\u1ea3ng 12,3% nam gi\u1edbi v\u00e0 8,8% n\u1eef gi\u1edbi c\u00f3 nhi\u1ec5m HBV m\u1ea1n. Tuy vi\u1ec7c ch\u1ee7ng ng\u1eeba HBV cho tr\u1ebb em t\u1ea1i Vi\u1ec7t Nam \u0111\u00e3 l\u00e0m gi\u1ea3m ph\u1ea7n n\u00e0o xu\u1ea5t \u0111\u1ed9 vi\u00eam gan virus B m\u1ea1n, nh\u01b0ng v\u1eabn \u0111ang c\u00f3 t\u00ecnh tr\u1ea1ng b\u00f9ng ph\u00e1t ung th\u01b0 bi\u1ec3u m\u00f4 t\u1ebf b\u00e0o gan li\u00ean quan \u0111\u1ebfn nhi\u1ec5m HBV t\u1ea1i Vi\u1ec7t Nam.<\/p>\n<h3><b>1.2. C\u00e1c y\u1ebfu t\u1ed1 nguy c\u01a1 ch\u00ednh<\/b><\/h3>\n<p>&#8211;\u00a0<b>HBV<\/b>: theo s\u1ed1 li\u1ec7u c\u1ee7a T\u1ed5 ch\u1ee9c Y t\u1ebf Th\u1ebf gi\u1edbi (WHO) n\u0103m 2016, d\u1ef1a tr\u00ean m\u1ed9t s\u1ed1 nghi\u00ean c\u1ee9u t\u1eeb 2003 \u0111\u1ebfn 2014, t\u1ec9 l\u1ec7 nhi\u1ec5m HBV \u1edf ng\u01b0\u1eddi l\u1edbn t\u1ea1i Vi\u1ec7t Nam kho\u1ea3ng 8,2-19%. Nhi\u1ec1u nghi\u00ean c\u1ee9u ph\u00e2n t\u00edch g\u1ed9p (meta-analysis) \u0111\u00e3 ch\u1ee9ng t\u1ecf nguy c\u01a1 ung th\u01b0 bi\u1ec3u m\u00f4 t\u1ebf b\u00e0o gan \u1edf nh\u1eefng ng\u01b0\u1eddi nhi\u1ec5m HBV cao h\u01a1n 15-20 l\u1ea7n so v\u1edbi nh\u1eefng ng\u01b0\u1eddi kh\u00f4ng nhi\u1ec5m. Nguy c\u01a1 b\u1ecb b\u1ec7nh trong cu\u1ed9c \u0111\u1eddi c\u1ee7a ng\u01b0\u1eddi nhi\u1ec5m HBV m\u1ea1n l\u00e0 kho\u1ea3ng 10-25%. C\u00f3 nhi\u1ec1u y\u1ebfu t\u1ed1 l\u00e0m t\u0103ng nguy c\u01a1 UTBMTBG \u1edf ng\u01b0\u1eddi nhi\u1ec5m HBV m\u1ea1n, bao g\u1ed3m c\u00e1c y\u1ebfu t\u1ed1 v\u1ec1 h\u00ecnh th\u00e1i (nam gi\u1edbi, tu\u1ed5i l\u1edbn, ti\u1ec1n s\u1eed gia \u0111\u00ecnh c\u00f3 ng\u01b0\u1eddi b\u1ecb ung th\u01b0 bi\u1ec3u m\u00f4 t\u1ebf b\u00e0o gan), v\u1ec1 virus (m\u1ee9c \u0111\u1ed9 nh\u00e2n b\u1ea3n HBV cao, ki\u1ec3u h\u00ecnh HBV, th\u1eddi gian nhi\u1ec5m HBV, \u0111\u1ed3ng nhi\u1ec5m v\u1edbi HCV, HIV, HDV), v\u1ec1 l\u00e2m s\u00e0ng (c\u00f3 x\u01a1 gan) v\u00e0 v\u1ec1 m\u00f4i tr\u01b0\u1eddng v\u00e0 l\u1ed1i s\u1ed1ng (ph\u01a1i nhi\u1ec5m v\u1edbi Aflatoxin, nghi\u1ec7n r\u01b0\u1ee3u n\u1eb7ng, h\u00fat thu\u1ed1c l\u00e1).<\/p>\n<p>&#8211;\u00a0<b>HCV<\/b>: theo s\u1ed1 li\u1ec7u c\u1ee7a T\u1ed5 ch\u1ee9c Y t\u1ebf Th\u1ebf gi\u1edbi (WHO) n\u0103m 2016, d\u1ef1a tr\u00ean m\u1ed9t s\u1ed1 nghi\u00ean c\u1ee9u t\u1eeb 2003 \u0111\u1ebfn 2014, t\u1ec9 l\u1ec7 nhi\u1ec5m HCV \u1edf ng\u01b0\u1eddi l\u1edbn t\u1ea1i Vi\u1ec7t Nam kho\u1ea3ng 1-3,3%. M\u1ed9t ph\u00e2n t\u00edch g\u1ed9p tr\u00ean m\u1ed9t s\u1ed1 nghi\u00ean c\u1ee9u b\u1ec7nh-ch\u1ee9ng (case-control studies) cho th\u1ea5y ng\u01b0\u1eddi c\u00f3 kh\u00e1ng th\u1ec3 kh\u00e1ng HCV c\u00f3 nguy c\u01a1 b\u1ecb ung th\u01b0 bi\u1ec3u m\u00f4 t\u1ebf b\u00e0o gan g\u1ea5p 17 l\u1ea7n so v\u1edbi ng\u01b0\u1eddi kh\u00f4ng c\u00f3 kh\u00e1ng th\u1ec3 kh\u00e1ng HCV.<\/p>\n<p>&#8211;\u00a0<b>\u0110\u1ed3ng nhi\u1ec5m HBV v\u00e0 HCV<\/b>: l\u00e0m t\u0103ng nguy c\u01a1 m\u1eafc b\u1ec7nh. Trong m\u1ed9t nghi\u00ean c\u1ee9u 24091 tr\u01b0\u1eddng h\u1ee3p ung th\u01b0 bi\u1ec3u m\u00f4 t\u1ebf b\u00e0o gan t\u1ea1i mi\u1ec1n Trung v\u00e0 mi\u1ec1n Nam Vi\u1ec7t Nam trong th\u1eddi gian 2010 \u0111\u1ebfn 2016, t\u1ec9 l\u1ec7 \u0111\u1ed3ng nhi\u1ec5m HBV v\u00e0 HCV l\u00e0 2,7%.<\/p>\n<p>&#8211;\u00a0<b>S\u1eed d\u1ee5ng \u0111\u1ed3 u\u1ed1ng c\u00f3 c\u1ed3n<\/b>: m\u1ed9t ph\u00e2n t\u00edch g\u1ed9p tr\u00ean 19 nghi\u00ean c\u1ee9u ti\u1ebfn c\u1ee9u ghi nh\u1eadn nguy c\u01a1 ung th\u01b0 bi\u1ec3u m\u00f4 t\u1ebf b\u00e0o gan t\u0103ng 16% \u1edf nh\u1eefng ng\u01b0\u1eddi s\u1eed d\u1ee5ng t\u1eeb 3 \u0111\u01a1n v\u1ecb \u0111\u1ed3 u\u1ed1ng c\u00f3 c\u1ed3n tr\u1edf l\u00ean m\u1ed7i ng\u00e0y v\u00e0 t\u0103ng 22% \u1edf nh\u1eefng ng\u01b0\u1eddi s\u1eed d\u1ee5ng t\u1eeb 6 \u0111\u01a1n v\u1ecb \u0111\u1ed3 u\u1ed1ng c\u00f3 c\u1ed3n tr\u1edf l\u00ean m\u1ed7i ng\u00e0y, v\u00e0 nguy c\u01a1 n\u00e0y c\u0169ng t\u0103ng ngay c\u1ea3 khi ch\u1ec9 s\u1eed d\u1ee5ng l\u01b0\u1ee3ng c\u1ed3n th\u1ea5p nh\u1ea5t m\u1ed7i ng\u00e0y (25g m\u1ed7i ng\u00e0y, t\u01b0\u01a1ng \u1ee9ng v\u1edbi 2 \u0111\u01a1n v\u1ecb \u0111\u1ed3 u\u1ed1ng c\u00f3 c\u1ed3n m\u1ed7i ng\u00e0y). \u1ede Vi\u1ec7t Nam, ch\u01b0a c\u00f3 s\u1ed1 li\u1ec7u ch\u00ednh th\u1ee9c v\u1ec1 m\u1ed1i li\u00ean quan gi\u1eefa ung th\u01b0 bi\u1ec3u m\u00f4 t\u1ebf b\u00e0o gan v\u00e0 vi\u1ec7c s\u1eed d\u1ee5ng \u0111\u1ed3 u\u1ed1ng c\u00f3 c\u1ed3n. Theo m\u1ed9t nghi\u00ean c\u1ee9u tr\u00ean 1617 b\u1ec7nh nh\u00e2n ung th\u01b0 bi\u1ec3u m\u00f4 t\u1ebf b\u00e0o gan t\u1ea1i mi\u1ec1n Trung v\u00e0 mi\u1ec1n Nam Vi\u1ec7t Nam, c\u00f3 68,6% b\u1ec7nh nh\u00e2n \u0111\u00e3 v\u00e0 \u0111ang s\u1eed d\u1ee5ng \u0111\u1ed3 u\u1ed1ng c\u00f3 c\u1ed3n v\u1edbi nhi\u1ec1u m\u1ee9c \u0111\u1ed9 kh\u00e1c nhau.<\/p>\n<h2><b>2. Ph\u00f2ng ng\u1eeba ung th\u01b0 bi\u1ec3u m\u00f4 t\u1ebf b\u00e0o gan<\/b><\/h2>\n<p>&#8211; N\u00ean s\u1eed d\u1ee5ng vaccin ch\u1ee7ng ng\u1eeba HBV cho tr\u1ebb em, nh\u1ea5t l\u00e0 tr\u1ebb s\u01a1 sinh \u0111\u1ec3 ng\u0103n ng\u1eeba vi\u1ec7c nhi\u1ec5m HBV. N\u00ean \u0111i\u1ec1u tr\u1ecb l\u00e2u d\u00e0i v\u00e0 hi\u1ec7u qu\u1ea3 cho c\u00e1c b\u1ec7nh nh\u00e2n nhi\u1ec5m HBV m\u1ea1n v\u00e0 c\u00f3 b\u1ec7nh l\u00fd gan \u0111ang ho\u1ea1t \u0111\u1ed9ng.<\/p>\n<p>&#8211; N\u00ean \u0111i\u1ec1u tr\u1ecb vi\u00eam gan virus C cho \u0111\u1ebfn khi b\u1ec7nh nh\u00e2n \u0111\u1ea1t \u0111\u01b0\u1ee3c \u0111\u00e1p \u1ee9ng virus b\u1ec1n v\u1eefng (sustained virologic response &#8211; SVR). Nguy c\u01a1 c\u1ee7a b\u1ec7nh s\u1ebd gi\u1ea3m \u0111\u00e1ng k\u1ec3 n\u1ebfu c\u00e1c b\u1ec7nh nh\u00e2n vi\u00eam gan virus C m\u1ea1n \u0111\u1ea1t \u0111\u01b0\u1ee3c SVR. Tuy nhi\u00ean, ngay c\u1ea3 khi \u0111\u1ea1t \u0111\u01b0\u1ee3c SVR, c\u00e1c b\u1ec7nh nh\u00e2n l\u1edbn tu\u1ed5i, s\u1ed1 l\u01b0\u1ee3ng ti\u1ec3u c\u1ea7u th\u1ea5p, v\u00e0\/ho\u1eb7c c\u00f3 x\u01a1 gan v\u1eabn c\u00f3 nguy c\u01a1 b\u1ecb b\u1ec7nh cao v\u00e0 c\u1ea7n \u0111\u01b0\u1ee3c t\u1ea7m so\u00e1t.<\/p>\n<p>&#8211; N\u00ean \u0111i\u1ec1u tr\u1ecb c\u00e1c b\u1ec7nh l\u00fd chuy\u1ec3n h\u00f3a nh\u01b0 b\u1ec7nh gan nhi\u1ec5m m\u1ee1 kh\u00f4ng do r\u01b0\u1ee3u (Non- alcoholic Fatty Liver Disease &#8211; NAFLD), b\u1ec7nh vi\u00eam gan tho\u00e1i h\u00f3a m\u1ee1 kh\u00f4ng do r\u01b0\u1ee3u (Non-alcoholic Steatohepatitis &#8211; NASH) v\u00ec c\u00e1c b\u1ec7nh l\u00fd n\u00e0y l\u00e0m t\u0103ng nguy c\u01a1 m\u1eafc b\u1ec7nh, nh\u1ea5t l\u00e0 khi \u0111\u00e3 c\u00f3 x\u01a1 gan. C\u00e1c h\u1ed9i ch\u1ee9ng chuy\u1ec3n h\u00f3a, \u0111\u1eb7c bi\u1ec7t l\u00e0 ti\u1ec3u \u0111\u01b0\u1eddng v\u00e0 b\u00e9o ph\u00ec, l\u00e0m t\u0103ng nguy c\u01a1 ung th\u01b0 bi\u1ec3u m\u00f4 t\u1ebf b\u00e0o gan tr\u00ean c\u00e1c b\u1ec7nh nh\u00e2n b\u1ecb NASH.<\/p>\n<h2><b>3. T\u1ea7m so\u00e1t ung th\u01b0 bi\u1ec3u m\u00f4 t\u1ebf b\u00e0o gan<\/b><\/h2>\n<p><b>3.1. \u0110\u1ed1i t\u01b0\u1ee3ng t\u1ea7m so\u00e1t<\/b><\/p>\n<p>&#8211; Nh\u00f3m nguy c\u01a1 cao: c\u00e1c tr\u01b0\u1eddng h\u1ee3p nhi\u1ec5m HBV m\u1ea1n v\u00e0 HCV m\u1ea1n ch\u01b0a \u0111i\u1ec1u tr\u1ecb, \u0111ang \u0111i\u1ec1u tr\u1ecb v\u00e0 \u0111\u00e3 ng\u01b0ng \u0111i\u1ec1u tr\u1ecb b\u1eb1ng thu\u1ed1c kh\u00e1ng virus, v\u00e0 c\u00e1c tr\u01b0\u1eddng h\u1ee3p x\u01a1 gan kh\u00f4ng li\u00ean quan \u0111\u1ebfn nhi\u1ec5m virus vi\u00eam gan.<\/p>\n<p>&#8211; Nh\u00f3m nguy c\u01a1 r\u1ea5t cao: c\u00e1c tr\u01b0\u1eddng h\u1ee3p x\u01a1 gan c\u00f3 li\u00ean quan \u0111\u1ebfn vi\u00eam gan do virus (HBV, HCV).<\/p>\n<p><b>3.2. Ph\u01b0\u01a1ng ti\u1ec7n v\u00e0 t\u1ea7n su\u1ea5t t\u1ea7m so\u00e1t<\/b><\/p>\n<p>&#8211;\u00a0\u00a0 T\u1ea7m so\u00e1t b\u1eb1ng si\u00eau \u00e2m b\u1ee5ng v\u00e0 x\u00e9t nghi\u1ec7m ph\u1ed1i h\u1ee3p c\u00e1c ch\u1ec9 d\u1ea5u sinh h\u1ecdc AFP, AFP-L3, PIVKA II.<\/p>\n<p>&#8211; T\u1ea7m so\u00e1t m\u1ed7i 6 th\u00e1ng \u0111\u1ed1i v\u1edbi nh\u00f3m nguy c\u01a1 cao v\u00e0 m\u1ed7i 3 th\u00e1ng \u0111\u1ed1i v\u1edbi nh\u00f3m nguy c\u01a1 r\u1ea5t cao.
