Nguy\u00ean t\u1eafc x\u1eed tr\u00ed \u0111au ng\u1ef1c<\/b><\/p>\n<p>&#8211; C\u1ea7n nhanh ch\u00f3ng \u0111\u01b0a ra ch\u1ea9n \u0111o\u00e1n x\u00e1c \u0111\u1ecbnh tr\u01b0\u1edbc khi quy\u1ebft \u0111\u1ecbnh \u0111i\u1ec1u tr\u1ecb tri\u1ec7u ch\u1ee9ng.<\/p>\n<p>&#8211; Ki\u1ec3m so\u00e1t h\u00f4 h\u1ea5p &#8211; huy\u1ebft \u0111\u1ed9ng: \u00f4xy li\u1ec7u ph\u00e1p, d\u1eabn l\u01b0u kh\u00ed n\u1ebfu tr\u00e0n kh\u00ed m\u00e0ng ph\u1ed5i. Cho thu\u1ed1c h\u1ea1 \u00e1p n\u1ebfu t\u0103ng huy\u1ebft \u00e1p, cho thu\u00f4c v\u1eadn m\u1ea1ch n\u1ebfu tru\u1ef5 m\u1ea1ch, t\u1ee5t huy\u1ebft \u00e1p, \u0111i\u1ec1u tr\u1ecb lo\u1ea1n nh\u1ecbp tim n\u1ebfu c\u00f3.<\/p>\n<p>&#8211; \u0110i\u1ec1u tr\u1ecb tri\u1ec7u ch\u1ee9ng: tu\u1ef3 theo m\u1ee9c \u0111\u1ed9 \u0111au ng\u1ef1c v\u00e0 nguy\u00ean nh\u00e2n g\u00e2y \u0111au, n\u1ebfu \u0111au ng\u1ef1c nhi\u1ec1u c\u00f3 th\u1ec3 cho thu\u1ed1c gi\u1ea3m \u0111au opioid ti\u00eam t\u0129nh m\u1ea1ch.<\/p>\n<p>&#8211; \u0110i\u1ec1u tr\u1ecb theo nguy\u00ean nh\u00e2n: ph\u1ee5 thu\u1ed9c nguy\u00ean nh\u00e2n m\u00e0 c\u00f3 ph\u01b0\u01a1ng ph\u00e1p \u0111i\u1ec1u tr\u1ecb kh\u00e1c nhau.<\/p>\n<p><b>3.2. X\u1eed tr\u00ed m\u1ed9t s\u1ed1 nguy\u00ean nh\u00e2n \u0111au ng\u1ef1c<\/b><\/p>\n<p><b><i>3.2.1. C\u01a1n \u0111au th\u1eaft ng\u1ef1c<\/i><\/b><\/p>\n<p>&#8211; D\u00f9ng s\u1edbm thu\u1ed1c ch\u1ed1ng ng\u01b0ng t\u1eadp ti\u1ec3u c\u1ea7u: aspirin (aspergic) 325-500 mg nhai ho\u1eb7c ti\u00eam TM. N\u1ebfu b\u1ec7nh nh\u00e2n c\u00f3 ch\u1ed1ng ch\u1ec9 \u0111\u1ecbnh d\u00f9ng aspergic c\u00f3 th\u1ec3 thay b\u1eb1ng clopidogrel 300 mg.<\/p>\n<p>&#8211; Gi\u1ea3m \u0111au ng\u1ef1c b\u1eb1ng nitrat v\u00e0\/ho\u1eb7c thu\u1ed1c ch\u1eb9n beta.<\/p>\n<p>&#8211; Ch\u1ed1ng \u0111\u00f4ng m\u00e1u b\u1eb1ng heparin ho\u1eb7c heparin tr\u1ecdng l\u01b0\u1ee3ng ph\u00e2n t\u1eed th\u1ea5p (LMWH).<\/p>\n<p><b><i>3.2.2 Nh\u1ed3i m\u00e1u c\u01a1 tim c\u1ea5p<\/i><\/b><\/p>\n<ul>\n<li><i>X\u1eed tr\u00ed ban \u0111\u1ea7u:<\/i><\/li>\n<\/ul>\n<p>&#8211; Cho b\u1ec7nh nh\u00e2n n\u1eb1m t\u1ea1i gi\u01b0\u1eddng, th\u1edf oxy v\u00e0 gi\u1ea3m \u0111au ng\u1ef1c b\u1eb1ng morphin ti\u00eam t\u0129nh m\u1ea1ch.<\/p>\n<p>&#8211; Nitroglycerin 0,4 mg ng\u1eadm d\u01b0\u1edbi l\u01b0\u1ee1i ho\u1eb7c natispray x\u1ecbt d\u01b0\u1edbi l\u01b0\u1ee1i. L\u01b0u \u00fd kh\u00f4ng d\u00f9ng nitroglycerin n\u1ebfu NMCT th\u1ea5t ph\u1ea3i ho\u1eb7c huy\u1ebft \u00e1p t\u1ed1i \u0111a &lt; 90 mmHg.<\/p>\n<p>&#8211; Ch\u1ed1ng ng\u01b0ng k\u1ebft ti\u1ec3u c\u1ea7u: Aspirin 325 &#8211; 500 mg nhai ho\u1eb7c ti\u00eam TM. N\u1ebfu b\u1ec7nh nh\u00e2n c\u00f3 ch\u1ed1ng ch\u1ec9 \u0111\u1ecbnh d\u00f9ng aspirin th\u00ec thay b\u1eb1ng clopidogrel 300 mg.<\/p>\n<p>&#8211; Ch\u1ed1ng \u0111\u00f4ng: heparin 65-70 UI\/kg ti\u00eam TM, sau \u0111\u00f3 duy tr\u00ec li\u1ec1u 15-18 UI\/kg\/h.<\/p>\n<p>&#8211; Thu\u1ed1c ch\u1eb9n beta: n\u1ebfu huy\u1ebft \u00e1p &gt; 90 mmHg v\u00e0 nh\u1ecbp tim &gt; 60 nh\u1ecbp\/ph\u00fat.<\/p>\n<ul>\n<li><i>\u0110i\u1ec1u tr\u1ecb t\u00e1i t\u01b0\u1edbi m\u00e1u:<\/i><\/li>\n<\/ul>\n<p>&#8211; T\u00f9y theo th\u1eddi \u0111i\u1ec3m b\u1ec7nh nh\u00e2n \u0111\u1ebfn vi\u1ec7n, m\u1ee9c \u0111\u1ed9 t\u1ed5n th\u01b0\u01a1ng, trang thi\u1ebft b\u1ecb hi\u1ec7n c\u00f3, t\u00ecnh tr\u1ea1ng c\u1ee7a b\u1ec7nh nh\u00e2n, c\u00e2n nh\u1eafc l\u1ef1a ch\u1ecdn ph\u01b0\u01a1ng ph\u00e1p \u0111i\u1ec1u tr\u1ecb t\u00e1i t\u01b0\u1edbi m\u00e1u b\u1eb1ng thu\u1ed1c ti\u00eau s\u1ee3i huy\u1ebft, can thi\u1ec7p \u0111\u1ed9ng m\u1ea1ch v\u00e0nh qua da ho\u1eb7c ph\u1eabu thu\u1eadt b\u1eafc c\u1ea7u n\u1ed1i ch\u1ee7 v\u00e0nh.<\/p>\n<p><b><i>3.2.3. Nh\u1ed3i m\u00e1u ph\u1ed5i<\/i><\/b><\/p>\n<ul>\n<li>Gi\u1ea3m \u0111au ng\u1ef1c b\u1eb1ng paracetamol. N\u1ebfu kh\u00f4ng \u0111\u1ee1 th\u00ec ti\u00eam morphin.<\/li>\n<li>Cho b\u1ec7nh nh\u00e2n th\u1edf oxy, nh\u1eb1m \u0111\u1ea1t \u0111\u01b0\u1ee3c SPO<sub>2<\/sub>\u00a0&gt; 90%.<\/li>\n<li>D\u00f9ng thu\u1ed1c ch\u1ed1ng \u0111\u00f4ng heparin, sau \u0111\u00f3 d\u00f9ng g\u1ed1i thu\u1ed1c ch\u1ed1ng \u0111\u00f4ng kh\u00e1ng vitamin K. Khi \u0111\u1ea1t \u0111\u01b0\u1ee3c INR trong kho\u1ea3ng li\u1ec1u \u0111i\u1ec1u tr\u1ecb t\u1eeb 2-3 th\u00ec xem x\u00e9t c\u1eaft heparin s\u1edbm.<\/li>\n<li>N\u1ebfu nh\u1ed3i m\u00e1u di\u1ec7n r\u1ed9ng c\u00f3 th\u1ec3 c\u00e2n nh\u1eafc d\u00f9ng thu\u1ed1c ti\u00eau s\u1ee3i huy\u1ebft ho\u1eb7c ph\u1eabu thu\u1eadt khi c\u00f3 r\u1ed1i lo\u1ea1i huy\u1ebft \u0111\u1ed9ng.<\/li>\n<\/ul>\n<p><b><i>3.2.4. Ph\u00ecnh t\u00e1ch \u0111\u1ed9ng m\u1ea1ch ch\u1ee7<\/i><\/b><\/p>\n<ul>\n<li>D\u00f9ng thu\u1ed1c gi\u1ea3m \u0111au, n\u1ebfu \u0111au ng\u1ef1c nhi\u1ec1u c\u00f3 th\u1ec3 ti\u00eam morphin.<\/li>\n<li>Ki\u1ec3m so\u00e1t huy\u1ebft \u00e1p v\u00e0 gi\u1ea3m nh\u1ecbp tim, t\u1ed1t nh\u1ea5t l\u00e0 d\u00f9ng nh\u00f3m ch\u1eb9n beta.<\/li>\n<li>H\u1ed9i ch\u1ea9n v\u1edbi b\u00e1c s\u1ef9 chuy\u00ean khoa ph\u1eabu thu\u1eadt m\u1ea1ch m\u00e1u x\u00e9t can thi\u1ec7p.<\/li>\n<\/ul>\n<p><b><i>3.2.5.
