D\u1ef1 ph\u00f2ng tr\u01b0\u1edbc ph\u01a1i nhi\u1ec5m HIV (PrEP) l\u00e0 g\u00ec?<\/h2>\n<p>PrEP hay Pre-Exposure Prophylaxis c\u00f3 ngh\u0129a l\u00e0 D\u1ef1 ph\u00f2ng tr\u01b0\u1edbc ph\u01a1i nhi\u1ec5m = D\u00f9ng ARV TR\u01af\u1edaC KHI ph\u01a1i nhi\u1ec5m HIV \u0111\u1ed1i v\u1edbi ng\u01b0\u1eddi c\u00f3 HIV \u00e2m t\u00ednh nh\u1eb1m d\u1ef1 ph\u00f2ng l\u00e2y nhi\u1ec5m HIV.<br \/>\n\u2022 PrEP l\u00e0 chi\u1ebfn l\u01b0\u1ee3c d\u1ef1 ph\u00f2ng HIV m\u1edbi b\u1eb1ng c\u00e1ch s\u1eed d\u1ee5ng thu\u1ed1c ARV \u0111\u1ed1i v\u1edbi ng\u01b0\u1eddi ch\u01b0a nhi\u1ec5m HIV.<br \/>\n\u2022 Theo T\u1ed5 ch\u1ee9c y t\u1ebf th\u1ebf gi\u1edbi (WHO) , PrEP l\u00e0 bi\u1ec7n ph\u00e1p d\u1ef1 ph\u00f2ng b\u1ed5 sung \u0111\u1ed1i v\u1edbi nh\u1eefng ng\u01b0\u1eddi c\u00f3 nguy c\u01a1 l\u00e2y nhi\u1ec5m HIV cao.<br \/>\n\u2022 PrEP c\u00f3 hi\u1ec7u qu\u1ea3 r\u1ea5t cao trong ph\u00f2ng l\u00e2y nhi\u1ec5m HIV: Gi\u1ea3m \u0111\u1ebfn 99% nguy c\u01a1 l\u00e2y nhi\u1ec5m qua \u0111\u01b0\u1eddng t\u00ecnh d\u1ee5c n\u1ebfu d\u00f9ng theo \u0111\u00fang ch\u1ec9 \u0111\u1ecbnh v\u00e0 V\u1edbi ng\u01b0\u1eddi ti\u00eam ch\u00edch ma tu\u00fd c\u00f3 t\u00e1c d\u1ee5ng b\u1ea3o v\u1ec7 \u00edt nh\u1ea5t 74%.<\/p>\n<h2>2. D\u1ef1 ph\u00f2ng tr\u01b0\u1edbc ph\u01a1i nhi\u1ec5m \u0111\u01b0\u1ee3c s\u1eed d\u1ee5ng cho ai?<\/h2>\n<p>PrEP s\u1eed d\u1ee5ng cho \u201cT\u1ea5t c\u1ea3 nh\u1eefng ng\u01b0\u1eddi CH\u01afA NHI\u1ec4M HIV m\u00e0 c\u00f3 NGUY C\u01a0<br \/>\nl\u00e2y nhi\u1ec5m HIV cao\u201d<br \/>\nC\u00e1c nh\u00f3m \u0111\u1ed1i t\u01b0\u1ee3ng r\u1ea5t hi\u1ec7u qu\u1ea3 v\u1edbi PrEP:<br \/>\n\u2022 Nam c\u00f3 quan h\u1ec7 t\u00ecnh d\u1ee5c\u00a0 v\u1edbi nam (MSM), Ng\u01b0\u1eddi chuy\u1ec3n gi\u1edbi n\u1eef (TG);<br \/>\n\u2022 C\u00e1c c\u1eb7p d\u1ecb nhi\u1ec5m (v\u1ee3\/ch\u1ed3ng\/b\u1ea1n t\u00ecnh th\u01b0\u1eddng xuy\u00ean c\u00f3 HIV d\u01b0\u01a1ng t\u00ednh).<br \/>\n\u2022 Ng\u01b0\u1eddi ti\u00eam ch\u00edch ma t\u00fay;<br \/>\n\u2022 Ph\u1ee5 n\u1eef m\u1ea1i d\u00e2m.<\/p>\n<h2>3. Ch\u1ec9 \u0111\u1ecbnh d\u1ef1 ph\u00f2ng tr\u01b0\u1edbc ph\u01a1i nhi\u1ec5m.<\/h2>\n<p>MSM v\u00e0 TG \u0111\u01b0\u1ee3c xem l\u00e0 c\u00f3 nguy c\u01a1 nhi\u1ec5m HIV n\u1ebfu h\u1ecd c\u00f3 \u00edt nh\u1ea5t m\u1ed9t h\u00e0nh vi nguy c\u01a1 trong 6 th\u00e1ng tr\u01b0\u1edbc:<\/p>\n<ul>\n<li>Quan h\u1ec7 t\u00ecnh d\u1ee5c qua \u0111\u01b0\u1eddng h\u1eadu m\u00f4n m\u00e0 kh\u00f4ng d\u00f9ng BCS.<\/li>\n<li>S\u1eed d\u1ee5ng ma t\u00fay \u0111\u00e1 (methamphetamine tinh th\u1ec3), ti\u00eam ch\u00edch ma tu\u00fd khi QHTD.<\/li>\n<li>B\u00e1n d\u00e2m.<\/li>\n<li>Nhi\u1ec1u h\u01a1n 1 b\u1ea1n t\u00ecnh.<\/li>\n<li>M\u1eafc nhi\u1ec5m khu\u1ea9n l\u00e2y truy\u1ec1n qua \u0111\u01b0\u1eddng t\u00ecnh d\u1ee5c (STI).<\/li>\n<li>\u0110\u00e3 t\u1eebng s\u1eed d\u1ee5ng PEP<\/li>\n<\/ul>\n<p>V\u1ee3\/ch\u1ed3ng\/b\u1ea1n t\u00ecnh c\u00f3 HIV \u00e2m t\u00ednh trong c\u1eb7p d\u1ecb nhi\u1ec5m \u0111\u01b0\u1ee3c cho l\u00e0 c\u00f3 nguy c\u01a1 b\u1ecb nhi\u1ec5m HIV n\u1ebfu:<\/p>\n<ul>\n<li>Quan h\u1ec7 t\u00ecnh d\u1ee5c qua \u0111\u01b0\u1eddng h\u1eadu m\u00f4n ho\u1eb7c \u00e2m \u0111\u1ea1o m\u00e0 kh\u00f4ng d\u00f9ng bao cao su.<\/li>\n<li>B\u1ea1n t\u00ecnh c\u00f3 HIV(+) ch\u01b0a \u0111i\u1ec1u tr\u1ecb ARV, ho\u1eb7c \u0111i\u1ec1u tr\u1ecb ARV d\u01b0\u1edbi 6 th\u00e1ng ho\u1eb7c \u0111i\u1ec1u tr\u1ecb tr\u00ean 6 th\u00e1ng m\u00e0 TLVR tr\u00ean m\u1ee9c ph\u00e1t hi\u1ec7n.<br \/>\n* N\u1ebfu B\u1ea1n t\u00ecnh c\u00f3 HIV(+) \u0111\u01b0\u1ee3c \u0111i\u1ec1u tr\u1ecb ART v\u00e0 \u1ed5n \u0111\u1ecbnh l\u00e2m s\u00e0ng (K=K hay \u0111\u01b0\u1ee3c bi\u1ebft l\u00e0 c\u00f3 t\u1ea3i l\u01b0\u1ee3ng virut HIV d\u01b0\u1edbi ng\u01b0\u1ee1ng ph\u00e1t hi\u1ec7n \u0111\u01b0\u1ee3c), V\u00c0 s\u1eed d\u1ee5ng bao cao su th\u01b0\u1eddng xuy\u00ean cho quan h\u1ec7 t\u00ecnh d\u1ee5c qua \u0111\u01b0\u1eddng h\u1eadu m\u00f4n v\u00e0 \u00e2m \u0111\u1ea1o th\u00ec PrEP kh\u00f4ng \u0111\u01b0\u1ee3c ch\u1ec9 \u0111\u1ecbnh.<\/li>\n<li>Ng\u01b0\u1eddi ti\u00eam ch\u00edch ma tu\u00fd, c\u00f3 s\u1eed d\u1ee5ng chung b\u01a1m kim ti\u00eam<\/li>\n<li>Ph\u1ee5 n\u1eef b\u00e1n d\u00e2m.<\/li>\n<\/ul>\n<figure id=\"attachment_1531\" aria-describedby=\"caption-attachment-1531\" style=\"width: 459px\" class=\"wp-caption alignnone\"><img decoding=\"async\" loading=\"lazy\" class=\"wp-image-1531\" src=\"http:\/\/noithathoaphat.in\/ctv\/wp-content\/uploads\/2023\/04\/download-3.jpg\" alt=\"minh-hoa-du-phong-truoc-phoi-nhiem-HIV\" width=\"459\" height=\"257\" \/><figcaption id=\"caption-attachment-1531\" class=\"wp-caption-text\">D\u1ef1 ph\u00f2ng tr\u01b0\u1edbc ph\u01a1i nhi\u1ec5m HIV<\/figcaption><\/figure>\n<h2>4. Ch\u1ed1ng ch\u1ec9 \u0111\u1ecbnh:<\/h2>\n<ul>\n<li>HIV d\u01b0\u01a1ng t\u00ednh<\/li>\n<li>C\u00f3 tri\u1ec7u ch\u1ee9ng ho\u1eb7c d\u1ea5u hi\u1ec7u nhi\u1ec5m HIV c\u1ea5p ho\u1eb7c c\u00f3 kh\u1ea3 n\u0103ng m\u1edbi nhi\u1ec5m HIV.<\/li>\n<li>\u0110\u1ed9 thanh th\u1ea3i creatinin \u01b0\u1edbc t\u00ednh &lt; 60 ml\/ph\u00fat.<\/li>\n<li>D\u1ecb \u1ee9ng ho\u1eb7c c\u00f3 ch\u1ed1ng ch\u1ec9 \u0111\u1ecbnh v\u1edbi b\u1ea5t k\u1ef3 thu\u1ed1c n\u00e0o trong ph\u00e1c \u0111\u1ed3 PrEP<\/li>\n<li>D\u01b0\u1edbi 35 kg<br \/>\nL\u01b0u \u00fd:<br \/>\n\u2022 Kh\u00f4ng ch\u1ec9 \u0111\u1ecbnh PrEP n\u1ebfu c\u00f3 ph\u01a1i nhi\u1ec5m v\u1edbi HIV trong v\u00f2ng 72 gi\u1edd qua. \u0110\u00e1nh gi\u00e1 v\u00e0 k\u00ea<br \/>\n\u0111\u01a1n d\u1ef1 ph\u00f2ng sau ph\u01a1i nhi\u1ec5m (PEP) v\u00e0 sau \u0111\u00f3 xem x\u00e9t ch\u1ec9 \u0111\u1ecbnh PrEP.<br \/>\n\u2022 Kh\u00f4ng c\u1ea7n ch\u1ec9 \u0111\u1ecbnh PrEP khi x\u00e9t nghi\u1ec7m t\u1ea3i l\u01b0\u1ee3ng HIV c\u1ee7a b\u1ea1n t\u00ecnh nhi\u1ec5m HIV \u0111ang \u0111i\u1ec1u<br \/>\ntr\u1ecb ARV &lt;200 b\u1ea3n sao\/ml v\u00e0 tu\u00e2n th\u1ee7 t\u1ed1t.<\/li>\n<\/ul>\n<h2>5. C\u00e1c h\u00ecnh th\u1ee9c s\u1eed d\u1ee5ng PrEP:<\/h2>\n<p>C\u00f3 2 h\u00ecnh th\u1ee9c ch\u00ednh s\u1eed d\u1ee5ng PrEP l\u00e0:<\/p>\n<ul>\n<li>PrEP h\u1eb1ng ng\u00e0y: d\u00f9ng cho m\u1ecdi \u0111\u1ed1i t\u01b0\u1ee3ng , s\u1eed d\u1ee5ng m\u1ed7i vi\u00ean m\u1ed9t ng\u00e0y, c\u00f3 t\u00e1c d\u1ee5ng b\u1ea3o v\u1ec7 t\u1ed1i \u0111a<\/li>\n<li>PrEP t\u00ecnh hu\u1ed1ng\u00a0 (ED PrEP): Ch\u1ec9 s\u1eed d\u1ee5ng cho nam c\u00f3 QHTD \u0111\u1ed3ng gi\u1edbi qua \u00a0\u0111\u01b0\u1eddng h\u1eadu m\u00f4n, T\u1ea7n su\u1ea5t QHTD trung b\u00ecnh \u00edt h\u01a1n 2 l\u1ea7n\/tu\u1ea7n v\u00e0 L\u00e0m ch\u1ee7 \u0111\u01b0\u1ee3c k\u1ebf ho\u1ea1ch QHTD.<\/li>\n<\/ul>\n<h2>6. C\u1ea7n u\u1ed1ng PrEP bao l\u00e2u th\u00ec b\u1eaft \u0111\u1ea7u c\u00f3 hi\u1ec7u qu\u1ea3?.<\/h2>\n<p>C\u1ea7n u\u1ed1ng PrEP m\u1ed9t th\u1eddi gian th\u00ec thu\u1ed1c \u0111\u1ea1t \u0111\u1ee7 n\u1ed3ng \u0111\u1ed9 b\u1ea3o v\u1ec7:<\/p>\n<ul>\n<li>V\u1edbi QHTD \u0111\u01b0\u1eddng h\u1eadu m\u00f4n, c\u00f3 th\u1ec3 b\u1eaft \u0111\u1ea7u b\u1eb1ng c\u00e1ch u\u1ed1ng 2 vi\u00ean c\u00f9ng m\u1ed9t l\u00fac v\u00e0 hi\u1ec7u qu\u1ea3 b\u1ea3o v\u1ec7 \u0111\u1ea1t \u0111\u01b0\u1ee3c sau 2-24 gi\u1edd.<\/li>\n<li>V\u1edbi QHTD qua \u0111\u01b0\u1eddng \u00e2m \u0111\u1ea1o v\u00e0 d\u00f9ng chung d\u1ee5ng c\u1ee5 ti\u00eam ch\u00edch, c\u1ea7n t\u1edbi 21 ng\u00e0y u\u1ed1ng PrEP li\u00ean t\u1ee5c th\u00ec m\u1edbi \u0111\u1ea1t \u0111\u1ee7 t\u00e1c d\u1ee5ng b\u1ea3o v\u1ec7.<\/li>\n<\/ul>\n<h2>7. T\u00e1c d\u1ee5ng ph\u1ee5 c\u1ee7a PrEP:<\/h2>\n<p>M\u1ed9t s\u1ed1 t\u00e1c d\u1ee5ng ph\u1ee5 c\u00f3 th\u1ec3 g\u1eb7p khi s\u1eed d\u1ee5ng PrEP:<\/p>\n<ul>\n<li>Ti\u00eau ch\u1ea3y;<\/li>\n<li>M\u1ec7t m\u1ecfi;<\/li>\n<li>\u0110au \u0111\u1ea7u;<\/li>\n<li>Bu\u1ed3n n\u00f4n.<\/li>\n<li>Ph\u00e1t ban (ph\u1ea3n \u1ee9ng d\u1ecb \u1ee9ng);<\/li>\n<li>V\u1ea5n \u0111\u1ec1 v\u1ec1 gan v\u00e0 th\u1eadn;<\/li>\n<li>T\u00ecnh tr\u1ea1ng vi\u00eam gan B n\u1eb7ng h\u01a1n;<\/li>\n<li>Nhi\u1ec5m axit lactic (qu\u00e1 nhi\u1ec1u axit lactic trong m\u00e1u).<\/li>\n<\/ul>\n<h2>8. PrEP: U\u1ed1ng \u0111\u1ebfn bao gi\u1edd?.<\/h2>\n<p>C\u1ea7n u\u1ed1ng PrEP \u0111\u00fang v\u00e0 \u0111\u1ee7 theo h\u01b0\u1edbng d\u1eabn th\u00ec m\u1edbi c\u00f3 hi\u1ec7u qu\u1ea3, nh\u01b0ng PrEP kh\u00f4ng ph\u1ea3i thu\u1ed1c d\u00f9ng c\u1ea3 \u0111\u1eddi. M\u1ed9t ng\u01b0\u1eddi c\u00f3 th\u1ec3 ng\u1eebng s\u1eed d\u1ee5ng PrEP v\u00ec m\u1ed9t s\u1ed1 l\u00fd do nh\u01b0:<\/p>\n<ul>\n<li>Kh\u00f4ng c\u00f2n nguy c\u01a1 cao nhi\u1ec5m HIV.<\/li>\n<li>Kh\u00f4ng th\u1ec3 nh\u1edb u\u1ed1ng thu\u1ed1c h\u00e0ng ng\u00e0y, hay qu\u00ean thu\u1ed1c. Tr\u01b0\u1eddng h\u1ee3p n\u00e0y ng\u01b0\u1eddi \u0111\u00f3 c\u00f3 th\u1ec3 xem x\u00e9t bi\u1ec7n ph\u00e1p d\u1ef1 ph\u00f2ng HIV kh\u00e1c.<\/li>\n<li>G\u1eb7p t\u00e1c d\u1ee5ng ph\u1ee5 n\u1eb7ng n\u1ec1 khi u\u1ed1ng PrEP v\u00e0 b\u00e1c s\u0129 quy\u1ebft \u0111\u1ecbnh d\u1eebng k\u00ea\u0111\u01a1n sau khi kh\u00e1m.<\/li>\n<\/ul>\n<h2>9.
